
New Client Steps
This section offers step-by-step instructions for new clients prior to their first visit.
Please watch the following video in it’s entirety for instructions on which sections of your chart to complete.
Step One: Create Your Client Chart
Please check your email for a link from Client Care to setup your new client chart.
The email will generate a username for you.
You will use that username to create a password.
Follow the prompts to setup your chart.
If you are unable to find the sign-up email from Client Care, please contact the office right away and let Charis know.
This link has a handy YouTube tutorial for how to setup and use your Client Portal. Please review prior to your first appointment.
Step Two: Fill Out Paperwork
Once your chart is setup, there are three sections that you will need to complete and submit prior to your first visit.
Contact Info Tab (please complete all boxes)
Health History (please include your current supplements in the Medications & Supplements sections!)
Forms & Records: there is a queue of PDFs for you to review and electronically sign. Please write down any questions you may have about specifics.
Step Three: Prepare Your Deposit
Please review the Financial Contract in the Forms & Records tab. There is a section within the contract for you to write in your personal payment plan. Please complete this section and come to your visit prepared with your $750 deposit. We accept cash, check, ACH payments, debit cards, HSA and credit cards.

Step Four: Your Initial Visit
Get ready for your initial visit with your midwife! We are so looking forward to working with you! At this visit we will review your paperwork, discuss pregnancy symptoms, take your vitals and listen for a fetal heart beat! We will discuss other introductory info regarding our practice and gift you with a new client binder and samples!
Please come hydrated as we’ll be drawing some initial labs at this visit, as well as asking you to leave a urine specimen. If you are planning to use your health insurance to pay for labs, please bring your Photo ID and Insurance Card with you to your visit.
